Allied Home Health Care Corporation has been serving patients in Orange County, San Bernardino County, Riverside County, and Los Angeles County for a near decade. We are pleased to expand our team and are currently seeking an experienced Licensed Vocational Nurse (LVN) Case Manager.
Education: Graduate of an accredited Nursing Program required.
Work Experience: Minimum of one (1) year of experience as a Licensed Vocational Nurses is required, preferably within the year prior to date of application.
Characteristic Duties & Responsibilities:

Provides professional nursing care utilizing all elements of the nursing process.
Implements orders for medications and treatments as prescribed by the physician and all aspects of the nursing care plan.
Initiates and applies appropriate preventative, therapeutic and rehabilitative nursing procedures in accordance with physician orders.
Monitors services delivered by home health care staff members.
Communicates patient assessment to appropriate individuals.
Maintaining patient confidentiality and quality care

Job Type: Full-time
Pay: $30.00 - $35.00 per visit
