Role Description

This is a part-time, on-site/remote role located in the Fort Worth, TX area. This requires 10-14 hours per week.

The Registered Dietitian will be responsible for:

· Providing nutritional counseling and medical nutrition therapy to clients.

· Develop, implement, and monitor individualized care plans for residents of long-term care facilities using medical nutrition therapy.

· Collaborate and assist with culinary team to ensure protocols in place, and adherence to federal and state guidelines are being followed.

· Preforms menu review and approval, sanitation audits and assistance with regulatory surveys.

· Attend interdisciplinary meetings and rounds with clinical staff, as appropriate.

· Conducts admission, annual, and quarterly nutritional assessments to identify nutrition concerns.

· Serve as the lead dietitian for assigned facilities.

· Working in collaboration with the healthcare team to provide appropriate care and follow-up for clients.

Qualifications

Dietetics and Nutrition skills
Experience with nutritional counseling and medical nutrition therapy
Weight Management skills
Excellent communication and interpersonal skills
Experience working with clients with chronic conditions, such as diabetes, heart disease, and renal disease
Must hold RD credential through CDR and be licensed in the State of Texas.
Ability to work collaboratively as a member of a healthcare team
