Registered Nurse - Home Health (up to 9-18 hours per week, Part-Time)

*Hybrid Position (In field with clients-Office Charting)

$95.00/visit-Start of Care

$65.00/visit - Recertifications

$55.00/visit - Skilled Nursing Visit

*Paid Training $30 per hour

Job Details:

Simply Moore Home Health is hiring a Registered Nurse (RN) to support clients within the Greater Indianapolis area and its surrounding counties. Registered Nurse candidate must have home health nursing experience. This nurse candidaten needs to be proficient with OASIS charting and use of EMR systems. This is a Part-Time HYBRID position workinng up to 24 hours per week/hours flexible. The nurse will complete home health visits as needed.

Requirements:

Current nursing license required for the state of Indiana
Degree in Nursing from an accredited nursing school, university or college
Valid drivers license and automobile insurance
Current in-person CPR certification
TB test screening (past 12 months)
One years Home Health based nursing experience (required)

OASIS experience (preferred)

Tech savvy, able to learn and navigate several platforms.
Perform routine duties independent of supervision but consults with others as required
Excellent communication and listening skills
Use EMR system
Excellent time management (required), tardiness not acceptable

Responsibilities:

Complete new patient intakes
Direct patient care (SOC's, ROC; Re-Certs, Discharge, Transfers, Nurse Visits and ALL OASIS documentation)
Document complex medical information to outline and improve health results that help determine immediate and routine care for the client
See patients and clients, as needed
Work collaboratively with the Administrator/Administrator Assistant
Communicate patient conditions and collaborate with the Administrator/Administrator Assistant to deliver care when patient needs evolve
Work to decrease re-admissions by promoting preventative care and ensuring continuity of care
Additional duties as assigned

Job Types: Part-time, PRN

Pay: $55.00 - $95.00 per hour

Expected hours: 8 – 16 per week
