The Case Manager is responsible to assist in the development, planning, coordination and administration of the activities of Utilization Review and Resource Management, including but not limited to daily review of medical records to determine appropriateness and medical necessity of admission, continued hospital stay and use of ancillary services
Position Requirements
- Florida RN License-active
- Case Management experience 2+ years
- Utilization review experience 2+ years
- Knowledgeable of resident’s financial status, diagnosis and discharge needs.
- Knowledgeable of criteria for Medicare, Medicaid, HMO and private insurance coverage.
- BLS certification through AHA or ARC
- set schedule required to include weekends
Position Purpose:
- Responsible to assist in the development, planning, coordination and administration of the activities of Utilization Review and Resource Management, including but not limited to daily review of medical records to determine appropriateness and medical necessity of admission, continued hospital stay and use of ancillary services
- Observe professional ethics in maintaining confidential information acquired concerning the personal, financial, medical, or employment status of patients of North Okaloosa Medical Center and their families.
- Must follow and enforce accepted safety practices for patients and hospital.
- Must participate in safety instructional programs.
- Facilitate patient case management in daily case management with Patient Case Managers, conducting the UR function, and prioritizing discharge planning.
- Teach nurses components of case management through formal and 1:1 teaching.
- Network with all healthcare professionals in facilitating patient discharge planning.
- Communicate numerous data to not only inhouse staff but also external agencies through us of fax machine, computer e-mail, copies of reports to be mailed.
