ORGANIZATIONAL SUMMARY A.Reports to: Clinical ManagerB.Supervises: Hospice AidesC.Key Working Relationships: Physicians, MercyOne North Iowa Medical Center and Hospice of North Iowa staffD.Primary Customer Groups: Patients, families, of Patients AgesESSENTIAL FUNCTIONS A.Provides competent ongoing holistic nursing assessments, planning, palliative interventions and evaluations, individualized patient and family centered care utilizing knowledge of diverse lifestyles, family structures, and dynamic cultural and religious practices, learning preferences, and age and gender specific considerations.B.Collaborates in the development, review and revision of interdisciplinary team plan of care.C.Oversees Hospice Aide services.D.Utilizes Hospice resources efficiently.E.Demonstrates flexibility to meet patient/family needs across MercyOne North Iowa Hospice.F.Able to manage caseload assigned by clinical manager.G.Demonstrates understanding of rules and regulations governing hospice services.H.Maintains accurate clinical record in timely manner and demonstrates competency with electronic medical record (EMR) and tablets.I.Participates in regularly scheduled meetings, in-services and quality assurance/improvement activities. Participates in Quality Assurance and Performance Improvement (QAPI) program.J.Demonstrates autonomy, professional judgment, responsibility, reliability, service excellence and critical thinking skills. K.Responds to on call duties as assigned.L.Must be able to function professionally in patient s residence where conditions may be less than desirable to individual nurse.M.Maintains required department confidentiality and abides by HIPPA regulations.N.Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.O.Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth and development and participates in medical center and/or departmental education programs.P.Communicates with and performs duties following the philosophy of Mercy Medical Center North Iowa and the Sisters of Mercy reflecting the organization s Mission, Values, and Guiding Behaviors.Q.Supports and abides by all medical center, departmental, and safety policies and procedures. R.Performs other responsibilities as requested by supervisor, manager or department head.ADDITIONAL FUNCTIONS POSITION QUALIFICATIONSA.Education formal : 1.Current RN Licensure in the state of Iowa, or qualified multistate licensure required Completion of physical assessment course preferred2.BSN preferred3.Hospice and Palliative Care Certification preferredB.Experience: 1.Must have demonstrated nursing competence in a health care setting for two years2.Caring for terminally ill patients/families, Hospice or home care nursing experience is preferredC.Special Skills and Competencies: 1.Able to work autonomously yet work within a team relationship2.Belief and knowledge of the hospice philosophy3.Ability to prioritize, plan, organize, implement, and coordinate patient care services4.Current motor vehicle driving license, motor vehicle insurance and availability to travel is required5.Availability to service patients in a variety of home settings including after business hours. 6.Obtain Port Access Certification within one year of hire date7.Completes the following mandatory courses (check all that apply)XMust provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.XCompletes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.Completes Advanced Cardiac Life Support (ACLS) American Heart Association within 6 months of hire and maintains current status.Completes Pediatric Advance Life Support (PALS) American Heart Association within 6 months of hire and maintains current status.Completes Neonatal Resuscitation Program (NRP) American Heart Association within 6 months of hire and maintains current status.Completes MANDT Training within 6 months of hire and maintains current status.Completes Trauma Nurse Core Course (TNCC) within 6 months of hire and maintains current status.Motor Vehicle: Will need to operate Mercy Vehicle X Personal Vehicle N/AFrequency of operation: X daily weekly _ monthly quarterly List any special licenses or requirements for Mercy vehicle operation NA MACHINES, TOOLS AND EQUIPMENT A.(List machines, tools and equipment that serve a major function of the job, use group titles of equipment, identify unique or specialized equipment.)
