Responsibilities The Intensive Community Wrap Around Supports program is designed in partnership with the Department of Developmental Services and the Department of Mental Health to create intensive, in-home and community wrap-around services to support adults between ages 18 and 30 with Autism Spectrum Disorder and Serious Mental Illness, or other significant mental health challenges. The goal of this program is to maintain individual stability and effective functionality for people served in their homes and communities by providing clinical services and coaching supports to identify and respond to barriers of obtaining stability and support individual progress while building skills and capacity for everyone.
Team Coaches will assist individuals in acquiring coping skills for anxiety, help develop self- advocacy, job readiness skills, awareness of public safety and develop tools for planning and executing functioning skills. They work with individuals served to increase interest and fluency in social and community activities and help them development independent living skills including personal hygiene, home maintenance, activities of daily living and other functional skills as assessed.
Other Key Responsibilities:
- Refer to community resources such as outpatient counseling and psychiatry. Assist individuals to obtain housing and entitlements.
- Help individuals secure and administer prescribed medications.
- Maintain communication with community team members and collaterals including psychiatrists, therapists, day program staff and others.
- Participate in the development of individualized treatment plans, Safety and Cope Ahead Plans with assigned individuals/families as clinically indicated. Assist in treatment planning towards the development of person centered, effective, and achievable treatment goals and objectives.
Qualifications- Bachelor's Degree in Human Services or related field, required.
- Minimum of five years' experience working with Autism Spectrum Disorder and significant mental illness, required.
- Driver's license, acceptable driving history and reliable transportation, required .
- Previous e xperience in assessment and treatment planning , preferred.
Compensation details: 57000 Yearly Salary
