Job Description
Under provider direction, the Nurse Coordinator provides skilled nursing care to patients in a variety of clinical settings. The Nurse Coordinator is responsible for coordinating all clinical functions, nursing practice, and the standards of care related to the delivery of patient care in a high volume outpatient facility and serves as a collaborative resource and program leader. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices, standards, protocols and guidelines. Works collaboratively with the physicians, APPs and administrative leadership teams to ensure successful implementation of new programs, services, staff education, training and quality projects.
- Performs clerical and clinical duties including patient education, cardiac stress testing, patient assessment and communication, staff training and routine competency assessments
- Demonstrates the technical skills and theoretical knowledge to meet the needs of the patient and family
- Assists with the daily coordination of patient care to facilitate efficient patient flow
- Delegates and supervises work as appropriate to ensure delivery of exceptional care
- Assists the health care team with the prevention of illness and injury, maintenance of wellness, and provision of comfort
- Performs other duties as assigned including the process of supporting the practice clinics.
Qualifications
- Experience in cardiology strongly preferred
- Current Connecticut RN License required
- Previous experience working in a medical facility in a coordinator or lead role preferred.
- Experience with medical software
