ResponsibilitiesThe On-call Registered Nurse is responsible for providing end-of-life care and hospice services to patients in their home or community setting. This includes intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care. As an On-call Registered Nurse you will:
- Assume primary responsibility for the patient/family caseload that includes assessing, planning, implementing, and evaluating care needs during on-call hours in collaboration with the triage team
- Report On-Call activities to the clinical leadership and Branch RN Case Managers at the end of the call shift
- Maintain and complete appropriate care coordination notes for visit notes, and other pertinent clinical information
- Assist, provide, or arrange for delivery of medications and supplies needed after business hours, weekends, and/or holidays
- As scheduled, attend interdisciplinary team meetings during on-call period for full team discussion, consultation and evaluation of patient/family needs
- Under the physician’s order, admits patients eligible for hospice services in collaboration with the triage team
- Reports patient status and need for other disciplines to clinical leadership, attending physician and hospice physician
- Develops patient care plan that specifically addresses identified patient problems; nursing problems and goals that has been identified as mutually agreed upon goals with patient(s) during on-call hours, weekends and/or holiday(s)
- Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
QualificationsRequired Knowledge, Skills and Experience
- Is currently licensed in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
- Current driver’s license and automobile in good working condition with proof of auto insurance
- Two years recent experience as a professional nurse
- Is able to meet certain physical demands of standing, bending, lifting, stooping or performing other work requiring light physical exertion (up to 30 pounds) on a continuous basis (over 50% of time); or moderate physical exertion (30 to 50 pounds) on a frequent basis (16 to 50% of time); or heavy physical exertion on an occasional basis (up to 15% of time)
- Ability to work in a field setting and exhibited ability to make sound nursing judgments; must be able to work independently
- Ability to assess patient needs and formulate individualized patient care plans to meet those needs
- Knowledge of physical, psychosocial and spiritual needs of terminally ill patients and their caregivers
- Effective written and verbal communication skills
Preferred Knowledge, Skills and Experience
- Two years’ experience within hospice
- Certified Hospice and Palliative Nursing
- Electronic Medical Records Experience
