RiverWoods Exeter is seeking a dynamic Nurse Practitioner to join our team and make a difference in the wellbeing of our residents of our communities. The ideal candidate will have a passion for building relationships with residents to help support them as they transition through the continuum of care. They will partner with residents and their families to provide peace of mind and care coordination as needed. The nurse practitioner is an integral part of an interdisciplinary team that works together to support our residents.

Responsibilities:
- Daily oversight of Wellness walk-in clinic for treatment of minor acute illness/injury
- Promotion and maintenance of health in collaboration with interdisciplinary team
- Promotion of individual optimal health and wellness plans
- Management of health care during acute phases of illness, including admitting residents to on site Healthcare Center in collaboration with clinical staff and Primary Care Physician
- Exercise of prescriptive authority according to New Hampshire statute
- Consultation and/or collaboration with other health care providers and community resources
- Referral to and from other health care providers and community resources
Qualifications:
- 3-5 years of experience working in family practice, walk in clinic, geriatric or a long-term care setting as a nurse practitioner
- Case management experience
- Must possess a current ARNP licensure in the State of NH
- Must be team oriented and be able to work in a fast-paced environment
