Triage Response Registered Nurse (RN) - Weekend Days
Location: Frankfort, KY (Serves Woodford, Franklin, Anderson & Owen counties)
Schedule: Saturdays & Sundays 9am-7pm
As a BCN Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness, but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient's care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

About the Role:
The Triage Response RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of Bluegrass Care Navigators. This position reports to the Executive Director and/or Clinical Nursing Officer.
Requirements:
- Is a graduate of an accredited school of professional (registered) nursing.
- Is currently licensed as an RN through the State Board of Nursing.
- At least one-year general nursing experience is preferred with current hospice, home health, medical, surgical, or critical care experience. Experience with providing good pain management, IV skills preferred. New nursing graduates will be considered along with other life experiences and interest in working in hospice field.
- Skilled in nursing practice, able to manage instances of elevated emotions or acute grief with professionalism; familiar and accepting of different cultures and lifestyles.
- Must be willing to work outside normal hours of operation and able to travel to patient homes during the specified on-call hours.
- Must possess strong critical thinking skills and be accustomed to providing care in the home setting autonomously.
- Must demonstrate the ability to work collaboratively with a variety of healthcare professionals, support staff and an interdisciplinary team.
- Skilled in collaborating with providers including physicians and allied health professionals to determine the appropriate plan of care for patients and families.
- Complies with accepted professional standards and principles.
- Understands hospice philosophy, principles of death/dying.
- Employees will be expected to comply with Bluegrass Care Navigators Employee Health, infectious disease, and vaccination policies.
- Must be a licensed driver in good standing with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Possesses and maintains current CPR certification.
- Have satisfactory references from nursing school, previous (or current) employers and/or professional peers.
- Have excellent observation, good nursing judgment and communication skills. Ability to provide compliant, detailed, clear written documentation at the point of service, consistent with Bluegrass Hospice Care standards.
- Is self-directed with the ability to work with little supervision.
- Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.
