Job Summary:
The Epilepsy Center in the Ayer Neuroscience Institute is highly dedicated to improving the lives of our patients. We believe good communication and mutual respect is at the heart of any good team, and is essential to providing the best care. Our level IV epilepsy center has all the diagnostic and treatment modalities available in order to provide cutting edge care for people with seizures. We look forward to welcoming a nurse navigator to assist us in coordinating the care and improving the efficiency with which the people of Connecticut can access these outstanding tools. We envision our nurse navigator as a central aspect of our program, interacting with patients, clinicians and staff across the continuum of care- starting with the first clinic visit and leading, in some cases, to brain surgery to eliminate seizures.
Job Responsibilities:
- Serves as a member of the multidisciplinary care team, functioning as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services.
- Navigates patients through the continuum of care, working with our social worker, faculty and staff to eliminate barriers, and enhance patient and colleague experience.
- Utilize electronic medical record reports and data management skills to coordinate patient flow through the epilepsy surgery program
- Identify patients with seizures within Hartford HealthCare who would benefit from referral into epilepsy clinics
- Participate as need in community and colleague education initiatives
- Assist in tracking and reporting of program performance measures, quality indicators, and contribute to the continuous improvement process.
- Maintains a minimum of 8 hours of epilepsy specific CEUs annually.
Qualifications
- BS in Nursing required. MS in nursing preferred.
- CT RN license required. Certification in specialty area preferred.
- Minimum 5 years clinical nursing experience preferred, particularly with neurological patients.
- Competent with standard software (Word, Excel, Electronic Medical Record) and willing and able to learn additional software as needed to enhance navigation and coordination responsibilities
- Knowledgeable regarding data interpretation and the tracking and evaluation of quality indicators
- Demonstrates use of clinical skills and expertise in the area of diagnosis, treatment, and psychosocial impact of disease.
- Demonstrates excellent customer service skills including superior telephonic communications skills with the ability to comprehensively assess patient’s physical and psychosocial status as well as conduct complex medical history discussions over the telephone.
- Ability to effectively and efficiently coordinate, implement and monitor the patient’s process of care, to communicate openly and empathetically with patients and their family members.
- Ability to work independently as well as collaboratively with navigation team and multidisciplinary care team in all aspects of defining and implementing navigation program.
- Ability to communicate effectively with diverse audiences including professional staff, patients, community, media, and other service agencies.
- Knowledge of adult learning competencies and patient teaching skills.
- Demonstrated management skills reflecting self-motivation, initiative and fiscal responsibility.
- Tolerance of ambiguity and the flexibility to respond to changing system needs.
- Demonstrates a commitment to own professional growth and competency.
