C.Demonstrates the ability to utilize critical thinking skills in the home environment and identify situations requiring physician notification or emergent care. D.Demonstrates the ability to care for clients with a variety of diagnosis including but not limited to orthopedic, surgical, medical, cardiac, wound/ostomy, and home infusion needs.E.Provides health coaching/education to patients/families and populations focused on achieving lower costs, engaged patients and reduced hospitalizations and ER/Urgent visit usage. F.Delivers nursing care for the clients in their homes, utilizing appropriate available resources.G.Involves the client/caregiver in goal development and plan of care development.H.Involves the client/caregiver in developing and carrying out the plan of care.I.Educates client/caregiver on disease process, treatment plan, and methods to increase independence for discharge planning. Evaluates and documents client's physical and emotional reactions to treatments, care, and medications in the electronic medical record (EMR).J.Conferences with other disciplines appropriately to provide an interdisciplinary approach to meet all identified client care needs.K.Assesses existing and potential problem areas, collects data, and reports significant information to the Clinical Manager or Clinical Leader.L.Functions purposefully in and adjusts to emergency situations as they arise.M.Instructs and supervises the home health aide, LPN, or other services provided to the client in their home requiring an RN supervision.N.Submits and maintains accurate, current, and complete documentation according to policy on a timely basis.O.Performs other responsibilities as requested by leadership. .P.Maintains required department confidentiality and abides by HIPPA regulations.Q.Provides competent patient care to the above indicated patient populations by obtaining and appropriately interpreting data, making appropriate decisions based on demonstrated knowledge of age-specific growth and development, and appropriately adapting procedures and treatments.R.Takes an active role in enhancing ability to carry out job functions through personal and professional/job-related growth and development and participates in medical center and/or departmental education programs. None.POSITION QUALIFICATIONSA.Education formal : 1.Current state Registered Nurse License or permit from State Board of Nursing required.2.Completion of ADN required BSN preferred. B.Experience: 1.Must have demonstrated competency in an acute patient care setting for at least one to two years.2.Home Health, community health, or hospice experience preferred.3.Previous experience with mental health patients preferred, but not requiredC.Special Skills and Competencies: 1.Current motor vehicle driving license, motor vehicle insurance and available to travel to client's residences required.2.Demonstrate the ability to use critical thinking skills and work in an autonomous setting.3.Computer and typing skills required.4.Must be able to read, write and/or speak English.5.Must be willing to drive in winter weather 6.Completes the following mandatory courses (check all that apply): XMust provide proof of Basic Life Support (BLS Provider-American Heart Association or Professional Rescuer-American Red Cross) prior to employment or within 30 days of hire and maintains current status.
