Job Requirements:
- FL RN (Registered Nurse) license
- Education: Associates Degree in Nursing
- Experience: Minimum of three years of Acute Care RN experience within a hospital setting required.
- Minimum 2 Years of RN Case Management within a Large Acute Care "Hospital" preferred.
- 1 year of Utilization Review experience preferred
About the Job:
- Working closely with the collaborative care team, the case manager assesses plans, intervenes, and reassesses an individual's needs to implement a plan of care to meet the needs of that individual
- The case manager assesses patient and family for clinical appropriateness for the setting and establishes plans for effective allocation of resources and management of the continuum of care.
- Utilization review is conducted according to ShorePoint Health guidelines
- Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge
- Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively.

- Minimum five years of clinical e experience in an acute care setting.
Preferred
- Previous case management and/or utilization management/utilization review experience.
