POSITION OVERVIEW: Adolescent Case Manager
Essential Functions: Adolescent Case Manager
- Ability to work to achieve the mission and goals of this agency.
- Create and implement groups that train youth in life skills - skills training in individual, family, and group work.
- Assess adolescent's need for and coordinate the client's health care, not limited to mental health and/or substance use treatment, medical, educational, social, or other services.
- Keep on-going records on all youth and families, including treatment goals, progress towards meeting goals, and case management documentation in the EHR.
- Develop appropriate linkages/referral systems for youth.
- Work as a liaison and resource for youth to bridge the gap between SLVBHG and community partners. Ability to work closely and cooperatively with external agency representatives.
- Work with clinicians at SLVBHG to provide on-going case management for adolescents enrolled in services.
- Provide support and linkage with external providers such as making appointments for the client if they are unable to do so.
- Work directly with youth and their families to provide psychosocial support.
- Identify and work with other community resources to provide prevention programming for youth.
- Ability to express oneself clearly and concisely, both orally and in writing
- Meet performance standards as specified by the supervisor and management staff.

Requirements:
Important Qualifications: Adolescent Case Manager
- Education: Bachelor's Degree
- Experience: One year of experience in related human service field preferred.
