Position Summary
To coordinate, implement, and plan nursing care to persons with breast health concerns, including the promotion of breast health in a timely and accurate manner.
Position Requirements
Formal Education / Training:
1. Licensed by the State of Tennessee.
2. Earned a nursing diploma or degree form a college or university
3. Breast Health Navigation Certification (credentialed) within 18 months of employment
Workplace Experience:
Coordination of Patient Care Delivery for Breast Health Services
Equipment and Skills Training:
Standard patient care equipment for services provided. The ability to work well under pressure, to function dependently and independently and to be flexible in applying basic nursing knowledge to a variety of setting. Equipment: Network Computer System
Physical Environment:
An Oncology medical/surgical unit and outpatient clinic whose patient population ranges consists of adults > 18yrs of age.
Physical Effort:
1. Able to communicate in English verbally and legibly.
2. Able to stand/work for up to eight hours a day.
3. Ability to travel when needed to area Breast Cancer functions.
3. Able to lift with assistance up to 50 lbs.
KEY RESULTS Performance Standards (Weight = 50%)
1. Plans, develops, and oversees the day-to-day clinical navigation activities for Breast Health Patients.
2. Initiates contact with breast patients and provides support to the patient in order to navigate the healthcare system.
3. Guides patients and families throughout their care as an open, knowledgeable and empathetic contact for all care needs.
4. Identifies health disparities and remove barriers to care.
5. Organizes appointments and explain the sequence of treatments to assure the patient complies with the treatment plans.
6. Provides providers to whom the patient is referred with timely data on patient progress.
7. Initiates referrals to connect patients with community resources and support systems.
8. Maintain documentation of activities to include capturing ongoing barriers, interactions, and barrier resolution on Breast Cancer patients throughout the cancer continuum.
9. Conducts breast health outreach activities and education to referrers, providers, other medical professionals, and to the corporate 'customer' community.
10. Assists with coordinating multidisciplinary Breast Cancer Conference and NAPBC programmatic activities as directed.
11. Assists in providing Breast Program metrics as needed for WMC Breast Program Leadership Committee.
12 Establishes and maintains positive working relationships with key internal and external customers.
