Position Summary:
Reporting to the Nursing Director, this position is responsible for planning, directing, coordinating, and providing high quality, individualized patient care using the components of the nursing process. On-call required; when on-call must arrive to the Hospital within 30 minutes.
Position Accountabilities:
The following are general job accountabilities:
Maintains current professional licenses and/or certifications as and when required by state, regulatory agency, or hospital mandate.
Demonstrates knowledge of, and adheres to, all applicable professional/regulatory practice acts, state/ federal regulations, and policies and procedures of San Ramon Regional Medical Center (SRRMC) and TENET CORPORATION.
Adheres to SRRMC practices for pain management with specific consideration of patient’s functional/developmental age, gender, cultural and/or personal preferences.
Identifies patients at risk for abuse as well as associated signs and symptoms. States mandatory reporting requirements for healthcare professionals.
Ensures patient safety by assessing and proactively implementing safety measures with specific consideration of patient’s functional and developmental age.
Provides appropriate nursing interventions. Competent in all routine nursing skills, including: (1) medication administration, (2) intravenous therapy, (3) management of indwelling catheters and drains, and (4) safe use of any medical equipment, such as glucose meters, IV pumps.
Functions in the capacity of Relief Charge Nurse per unit Charge Nurse guidelines, when assigned. Ensures that the operations and nursing care follow Medical Center and department policies and standards. Keeps Nursing House Supervisor and/or Manager/Director informed when difficulties and/or unusual situations occur. Assigns and provides relief for allotted meal and break periods for staff. Supervises safe patient care when float RNs, travel RN’s or orientees are on the unit.
Demonstrates the ability to provide care in emergency situations and follows established emergency procedures and protocols.
Delegate’s patient care tasks to ancillary personnel based on individual competency and scope of practice. Evaluates effectiveness of care performed.
Provides compassionate and caring support to families.
Determines the presence and type of Advance Directive, as appropriate. Contacts appropriate resources to assist the patient and family as needed. Completes accurate and timely documentation of all nursing care provided to patients per policy, including an evaluation of patients’ response to medications, treatments and procedures.
Completes computer documentation including: barcode scanning, management of computerized orders, the hybrid chart, and entering charges.
Accesses facility information systems to retrieve and utilize pertinent information for the provision of patient care or in the performance of other job related responsibilities. Follows Medical Center’s policy and procedure as they pertain to all hospital information systems.
Practices Standard Precautions while providing patient care and performing other hospital services.
Demonstrates the knowledge and skills to safely and appropriately fulfill job specific responsibilities related to safety, security, emergency preparedness, control of hazardous materials/waste, life safety, medical equipment and utility systems as outlined in the hospital’s environment of care plan.
Actively supports the organization’s mission, vision, values and goals. Participates in activities within the organization that support performance improvement.
Completes mandatory TENET ethics program. Conducts all hospital business within the highest standard of ethical behavior.
Assures that the rights of all patients are respected and maintained by allowing for privacy, confidentiality, and dignity in the provision of service.
Protects hospital resources through appropriate and careful use of supplies and equipment.
Maintains required applicable education and in-service requirements, timely acknowledgement of unit communication. Participates in departmental staff meetings attending 70% of the yearly meetings and signing off staff meeting minutes if did not attend.
Communicates and follows the organization’s chain of command for notification of patient care and/or service issues.
Expresses and accepts feedback in a professional manner.
Initiates a multidisciplinary, goal oriented care plan for each patient in collaboration with members of the health care team and provides care based on that plan.
Demonstrates the ability to organize workload, set priorities, and use time efficiently to complete work on time and in an organized manner and clock out timely.
Demonstrates reliability as evidenced by attendance records and punctuality.
Consistently appears in attire appropriate to the professional environment.
Completes .edu’s and other required hospital education by the due date.

Complies with San Ramon Regional Medical Center Policies/Procedures protecting patient information and the confidentiality of the information in accordance with the Federal and State regulations.
The following are unit-specific accountabilities:

Performs duties in a manner that provides for the psychosocial and spiritual needs of the patient and family. Completes all assessments in a timely manner. Incorporates age-specific considerations, including functional and developmental age, into the care of the neonate (0 to 30 days), adolescent (12 to 17 years) and adult (18 to 70 years) patients.

Effectively manages pain control in the laboring patient by (1) assisting the anesthesiologist with epidural catheter placement and (2) monitoring fetal and maternal status during catheter placement and after medication administration, medicating to appropriate pain scale.

Provides patient education related to infant security measures during hospitalization, such placement of infant security device on the neonate.

Provides routine pre- and post-operative, labor, delivery and mother/baby nursing care to include (1) assessment of fetal and maternal well-being, (2) amniotic fluid status, (3) vaginal bleeding, (4) recognizing and appropriately managing abnormalities, (5) cervical examination, (6) Apgar scoring, (7) meconium aspirations, and (8) insertion of fetal scalp electrodes and subsequent fetal monitoring. Collaborates care with obstetrician, anesthesiologist and pediatrician.

Creates and maintains a sterile environment for Cesarean sections as scrub or circulating nurse, assuring maintenance of all emergency equipment, including anesthesia and crash carts, and assisting physicians with deliveries.

Demonstrates the ability to provide care in emergencies and follows established emergency procedures and practices. Recognizes and responds appropriately to maternal and fetal distress. Assists physician with resuscitation as required.

Minimum Experience/Skills:
Minimum of one (1) year recent experience within the last three (3) years of Full –Time high risk Labor & Delivery experience;
OB triage and Circulating vaginal & C-section deliveries; (will train to scrub C-sections and train to postpartum/well baby if no experience).
Licenses/Certificates/Credentials:
Current California RN license
Current BLS certification (AHA certified)
Current NRP certification
AWHONN Intermediate Fetal Monitoring is a requirement within 1 year of hire, unless documentation of completion is provided prior to hire
AWHONN progressive Fetal Monitoring classes are recommended every 2 years
