Monadnock Family ServicesDescription:
Monadnock Family Services (MFS) seeks a Critical Time Intervention (CTI) Transition Coach who will partner with clients and community service providers to identify barriers and enact comprehensive plans with appropriate connections to support stability and safety in the community. Client eligibility consists of a single factor, namely that that individual is ready for discharge from an institutional setting (DHHS may partner with Provider Agencies to identify additional disqualification profiles).
Over the 9-month (plus pre-CTI) service lifespan, service touchpoints decrease, with most of the time and effort committed to phase 1, where service linkages are made and primary presenting social determinants of health (SDoH) needs (like housing and entitlements) are addressed. The intervention end at the 9-month mark, culminates in graduation and full transition to the community resources that were strengthened throughout the life of the CTI program.
The CTI Transition Coach will be responsible for up to twenty (20) people at a time.
This role will be supervised by the CTI Transition Supervisor and is community and office based with remote opportunities as needed. Travel is anticipated at 50%.
Requirements:
- Bachelor's degree
- Needing DHHS approval: Associates Degree or High School Diploma with sufficient (TBD) work experience
- Experience with behavioral health SDoH systems, including case management or delivery of clinical intervention.
- Experience with social determinants of health-related programs, including but not limited to housing, employment, food insecurity, and entitlements.
- Understanding of NH systems of care, with experience working between systems.
- Comfort with technology and reporting systems, with understanding that use of the agency's electronic record is compulsory.
