Case Manager with BA for SLV Behavioral Health Group
We are seeking a highly motivated and compassionate Case Manager for Mental Health to join our team. The ideal candidate will have a Bachelor's degree in a related field and experience working with individuals with mental health conditions.
Responsibilities: Case Manager for Behavioral Health
- Conduct assessments and develop individualized care plans for clients with mental health conditions
- Provide ongoing support and advocacy for clients to ensure they receive appropriate services and resources
- Collaborate with healthcare providers, social workers, and other professionals to coordinate care for clients
- Monitor and evaluate client progress and adjust care plans as needed
- Maintain accurate and up-to-date client records and documentation
- Participate in team meetings and case conferences to discuss client progress and treatment plans
Requirements: Case Manager for Behavioral Health
- Bachelor's degree in social work, psychology, or a related field
- Experience working with individuals with mental health conditions
- Knowledge of mental health diagnoses and treatment options
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Excellent organizational and time management skills
- Proficiency in Microsoft Office and electronic health record systems
