JOB SUMMARY
The Director of Nursing, reporting to the Health Care Administrator, directs and oversees care for all residents of Williamsburg Landing Health and Rehabilitation Center, ensuring compliance with state and federal laws, licensing, and accreditation (CARF-CCAC) regulations. The Director is responsible for establishing departmental performance goals and recommending policies to carry out its mission.
ESSENTIAL JOB FUNCTIONS
- Develops and maintains nursing service objectives, standards of practice, policies and procedures, in collaboration with the Health Care Administrator and the facility Medical Director
- Establishes work processes and ensures adequate resources to include equipment, supplies, and staffing to ensure residents receive quality care
- Participates and oversees the recruiting, interviewing, and evaluation of staff; supports the Shift Supervisors and Nurse Educator in their efforts to coordinate the onboarding process including the recommendation and coordination of staff training
- Understands and communicates the position control for Health & Rehabilitation to ensure appropriate vacancies are being advertised; updates the shared documents in real time to represent the most accurate position control
- Coordinates that all worker's compensation matters are processed timely and accurately prior to submission to Talent Management
- Co-investigates with the Shift Supervisors and responds to areas of opportunity identified through We Care Connect and employee Stay Interviews
- Rounds daily on resident floors to observe nursing practices and ensure resident satisfaction; reports findings to Health Care Administrator
- Assists in the development of the annual departmental operating and capital budgets
- Tracks employee turnover and assumes shared responsibility for same; implements initiatives to reduce employee turnover
- Represents resident interests in multi-disciplinary meetings, ensuring care plans are maintained through collaboration with Therapy, Activities, Social Work, and physicians
- Under the direction of the Health Care Administrator and in coordination with the Shift Supervisors, develops and conducts a formalized Quality Assurance Performance Improvement (QAPI) program
- Recommends and completes plans of correction for past non-compliance scenarios; involves CNAs and Nurses in Action Plans
- Leads weekly quality meeting ensuring concerns regarding resident weights, readmissions, wounds, behaviors, falls, usage of antipsychotic medication, pain levels are addressed; if a concern is raised, leads a formal QAPI action plan process
- Performs audits of medical records, identifying procedural deficiencies and ensuring appropriate corrective action is taken; ensures that the appropriate documentation is present for skilled residents to ensure HRC receives payment for services rendered
- Takes the lead with the Health Care Administrator and Shift Supervisors along with Security and/or local law enforcement in investigations of elder abuse, neglect, exploitation and misappropriation
- Assumes on call duty in a regular rotation and assigns the on call schedule to assure 24 hour coverage by nursing administration
- Maintains positive interactions with residents, guests, and other Williamsburg Landing employees; monitors resident surveys to improve residents' perception of service quality
- Coordinates and communicates with the Shift Supervisors to ensure staff receives all required trainings and encourages participation in professional development opportunities
- Holds a regular meeting with the Medical Director and other providers to discuss areas of opportunity and clinical initiatives
- Conducts meetings at least monthly with nursing team members to discuss areas of opportunity and updates in community events
- Oversees the completion of annual performance evaluations with the Shift Supervisors for all nursing team members; ensures that these reviews are completed on time
- Holds nursing team accountable for essential responsibilities and issues timely Employee Disciplinary Notices (EDNs) when necessary
- Coordinates care provided by contracted providers i.e. Wound Physician, Psychiatric Nurse Practitioner, Podiatrist, Dentist; ensures documentation from visits are uploaded into the EMR
- Inputs quality data into the monthly COR spreadsheet and ensures that reporting managers input data by the due date; leads the COR call
- Reviewed the CASPER quality reports on a monthly basis with the MDS Coordinator for areas of opportunity; if identified, creates a QAPI action plan
- Coordinates with the MDS representative to ensure timely transmission of the MDSs
- Participates in committees as requested
- Performs other duties as assigned
QUALIFICATIONS
Current Commonwealth of Virginia Registered Nurse licensure required with a minimum of two (2) years' experience in long term care facility supervision
KNOWLEDGE, SKILLS, AND ABILITIES
Comprehensive knowledge of geriatric nursing standards, principles, and practices; thorough practical knowledge of state and federal regulations related to nursing homes; knowledge of Medicare regulations; comprehensive knowledge of resident care standards, psychosocial dimension of gerontology, and medical records
