Physical Therapist (PT)
POSITION SUMMARY:
You will help assist patients in restoring function, relieving pain, and preventing disability. You will provide preventive care, rehabilitation, and treatment for patients with chronic conditions, illnesses, surgery, or injuries while keeping detailed documents on patient progress
RESPONSIBILITIES:
Conduct an initial evaluation/assessment and report findings to the physician and Clinical Manager and develop the plan of care promptly for patients
Perform physical therapy procedures, measurements, evaluations, and diagnostic tests on patients in the home
Document initial evaluation and treatment plan and re-evaluations of patient's progress every thirty (30) days and update plan of care as necessary
Complete comprehensive assessments and OASIS data, as indicated on a timely basis
Assist with orientation, supervision, and consultation services to Physical Therapy Assistants
Observe, record, and report to the Health Care Provider the patient's treatment, response, and progress and promptly report significant changes and obtain orders to revise the plan of care
Communicate with the Health Care Provider and other members of the health care team for the delivery and coordination of quality physical therapy services
Other duties as assigned
QUALIFICATIONS:
Master's degree in Physical Therapy
Active Physical Therapy license in the state of practice and in good standing
At least one year of experience working as a Physical Therapist
Must have a vehicle, a valid current driver's license, and automobile insurance
Ability to make home visits and provide physical therapy evaluations and treatments in the home environment, complete documentation effectively, and submit in a timely manner
Home Health and Home Care Home Base documentation experience
