SUMMARY
The Quality Care Coordinator functions as part of the Value-Based Care/Quality Team. This position will assist patients and physicians within the University Medical Group, LLC (UMGIPA). UMGIPA is an integrated group of Primary Care physicians across various healthcare organizations in the New Orleans/Metro area. By being clinically integrated, the goal of UMGIPA is to enhance patient outcomes while decreasing healthcare costs. Instead of performing direct patient care, the Quality Care Coordinator will serve as a liaison between the patients, caregivers, providers, and the Value-Based Care team within UMGIPA. While the main location for this position is the LSU Healthcare Network, traveling to various UMGIPA clinics across the New Orleans/Metro area will likely occur.
Business Hours: Monday through Friday**No nights, weekends or major holidays**

Job Responsibilities
- The Quality Care Coordinator is responsible for assisting and guiding the patient throughout their entire care experience
- Schedules various types of patient appointments such as routine clinic visits, Annual Wellness visits, Post-Hospital Discharge visits, ancillary services, lab testing, etc.
- Calls various lists of patients for specific HEDIS metrics in order to close gaps in care and increase STAR/RISK scores
- Assists the Director of Quality in developing and implementing effective care management protocols and workflows to enhance patient care and outcomes
- Performs manual chart audits across various EMRs to extract possible compliant records to submit to UMGIPA and close gaps in care
- Works with the Director of Quality and third-party billing company to address any issues with coding and/or supplemental claims
- Contacts and follows up with patients for blood pressure checks, lab work, cancer screenings, etc.
- Serves as the main point of contact for UMGIPA patients and caregivers
- Conducts pre-visit planning by documenting care/coding reminders and/or notes for the physicians prior to the patient's appointment
- Communicates with the patient's provider directly – as applicable
- Assists patients with different needs such as medication refills, ordering referrals, lab results, etc.
- Ability to travel to multiple UMGIPA clinic locations as applicable
Requirements:
EDUCATION and/or EXPERIENCE
- High school diploma or equivalent required
- Successful completion of an accredited LPN program required
- Minimum 1 year of clinical experience required
