JOB DESCRIPTION
SUMMARY
To provide vision care in the diagnosis and treatment of patients of the PATHS SchoolBased Clinic and PATHS Medical Offices. Responsible for providing comprehensive vision care that is consistent with accepted professional standards to children ages from kindergarten to twelfth grade as well as adults. Expected to consistently work to ensure patient satisfaction and maintain positive relationships with the PATHS' staff and school personnel. Works as part of a care team that practices evidenced based medicine in aPatient Centered Medical Home model of care.
Education and/or experience
Doctor of Optometry degree (OD)
Competencies
Minimum of one years' experience in human/social services, preferably in a healthcare environment.
Training or direct experience in counseling, interviewing, service plan development, decision making, and information and referral services is preferred.
Essential Functions

1. Provide general and comprehensive vision services to all ages.
2. Assist with the development, implementation and monitoring of vision policies and procedures.
3. Conduct routine eye exams, diagnose and evaluate vision conditions and maintaining accurate records to support diagnose/evaluation.
4. Prescribe medications to treat ocular diseases and disorders.
5. Promote eye health by teaching patients about proper eye care techniques.
6. Assist with the development and implementation of Vision Quality Assurance.
7. Train and evaluate assigned staff.
8. Other assignments by Chief Medical Officer as necessary.
