Summary:
The Nurse (LPN) is part of the Assertive Community Treatment (ACT) OR Family Practice team that provides person-centered services addressing a breadth of beneficiary’s needs, helping him or her achieve their personal goals.
Essential Duties and Responsibilities:
- Manages the medication system in conjunction with the psychiatrist, administers and documents medication treatment;
- Build rapport and engagement with beneficiaries
- Engage in health promotion, prevention and education activities;
- If the beneficiary is in agreement, develop strategies to maximize taking medications as prescribed (e.g., reviewing home environment to find cues to remind beneficiary to take medications; work with beneficiary and psychiatric care provider to scale back the number of times medications are taking each day);
- Communicating and coordinating services with other medical providers;
- Educating the team in monitoring psychiatric symptoms and medication side-effects;
- Provide “first responder” crisis response to any beneficiaries experiencing a crisis;
- Attending daily ACT team meetings and actively participating;
- Preparing oral medications for delivery and administering injectables as prescribed.
- Collaborating with the supervising RN and ACT staff to complete tasks within the proper scope of practice;
- Accurately documents all billable encounters into Southeastern Integrated Care’s EMR (electronic medical record) system within 24 hours. Any corrections will be entered within 24 hours of being notified.
- Other duties as assigned by RN Supervisor.
Supervisory Responsibilities:
The Nurse (LPN) position does not oversee staff.

Education/Experience
- Minimum one year of experience working with adults with serious mental illness;
- Diploma for (LPN)
Required Skills/Abilities
- Working knowledge of psychiatric medications;
- Ability to empathetically interact with beneficiaries and their families;
- Strong clinical skills;
- Operate independently or with minimal supervision.

- Valid NC Nursing License or Compact License that allows practice in NC;
- Copy of a valid, unrestricted NC Driver’s license, proof of insurance
