The transitional programs of AOASCC provide vital support to individuals with disabilities and frail older adults, enabling them to remain safely in their own homes. As a transitional coordinator, you will play a crucial role in providing support to individual clients within the community and specialized care managers, ensuring the continued quality of care for our clients. Responsibilities:
As a Transitional Program Coordinator, you will be crucial in coordinating services and providing client support and guidance. Your work will enable them to remain in the community and avoid unnecessary hospitalization or institutional placement, making a significant difference in their lives. You will register service referrals, determine volunteer assignments, conduct follow-up surveys, and ensure program compliance].
Assist the Program Coordinator in planning and administering relevant programs.
Qualifications:
- Bachelor’s degree in a related field (e.g., social work, gerontology, public health).
- Experience working with older adults, individuals with disabilities, and caregivers.
- Strong organizational, communication, and problem-solving skills.
