Summary:
- The RN/LPN will be responsible for providing professional, evidence-based client-centred care for individuals staying in the Hospice Residence and or patient home including all aspects of client and family care.
- The RN will be holistic, careful, and ethical in interactions with clients, families and colleagues, and make sound judgments based on experience and knowledge.
- Reports to Supervisor of Unit or MD.
- Duties and Responsibilities: Assume a lead role with the interdisciplinary team.
- Part of a care delivery team consisting of LPN, Social Worker, CNA, Spiritual Advisor and MD.
- Provide symptom and pain control therapeutic treatment Complete admission assessments and care-planning across the domains of physical and psychosocial care Provide client care including bathing, dressing, feeding, assist to bathroom, physical therapy as directed Critical thinking skills to recognizes changes in condition or mentation.
- The RN will liaise with the Physician/Nurse Practitioner and other Health Care Workers to support the client and family.
- Provide medication as prescribed by physician Proficient in contacting authorities on patient demise and notification of MD Experience & Education.
Requirements:
- Graduate of an accredited school of nursing, License in good standing, One year of Experience in Hospice AHA BLS required.
