Responsibilities Include (though are not limited to)
- LVN Nurse will be measuring residents' vital signs daily, or as needed, notify MD in case of abnormality.
- LVN Nurse will be responsible for the safety and well-being of residents (update appraisal needs and follow up with DON to ensure consistency and accuracy).
- LVN Nurse you will provide care and supervision to residents (vital sign measure, Injections, skin checks, medication & incontinence management, communication with health care professionals, responsible parties and administrators, diabetes control).
- LVN Nurse assists with medication storing and distribution.
- LVN Nurse duties will include inspecting of the eMAR to keep up to date at all times, including but not limited to order medications, communicate with pharmacy and/or MD, discontinue medications, etc.
- Assist in continuous observation of residents in regard to their physical/mental/emotional well-being.
- Document in EHR and eMAR all incidents, assessments and care plans of residents.
- Check, create and submit all incidents and death reports to the Department of Social Services.
- Communicate and follow-up with responsible parties to stay in compliance with MD orders.
- Assist, train and supervision of caregivers for delivering quality ADL's.
- Assist leads with any assignments or work requests.
- Conduct training in-services of care staff, when needed.
- Report any suspicion or evidence of elder abuse, mistreatment or neglect.
- Assist in keeping residents' and medication rooms' areas neat and clean.
- Any tasks assigned, as needed.
Requirements:
- Must be authorized to work in the US.
- Able to speak, read and write in English.
- First-Aid, CPR, and/or BLS certified.
- Graduate from an accredited school of nursing.
- Must have proof of LVN License in the State of California and in good standing.
- One (1) year of experience in caring for patients in a residential setting, health care or senior services setting.
- One (1) year of experience in managing and/or supervising others within the care team.
- Experience with care and supervision for elders and seniors (6 months or more experience in health care preferred).
- Must be able to successfully complete Basic Life Support certification and bloodborne-pathogens training within 30 days of hire
- Physical requirements include but are not limited to: able to lift up to 40 lbs., push/pull up to 20 lbs., carry up to 10 lbs., frequent standing and walking required
- Reliable transportation
- Ability to follow basic instructions and carry out assignments independently.
- Ability to interface appropriately with other staff, residents, family members, visitors and outside contractors.
- Ability to work flexible hours including and not limited to: weekends, evenings, holidays, and/or events.
- Own a smart phone (for schedule updates and communication).
- Possess a means of reliable transportation.
- Valid California driver's license, good driving record with active auto insurance.

$32.00 - $37.00 per hour
