Dietitian, Fresenius Management Services, Inc., a Fresenius Medical Care N.A. company, Columbus/Greensburg, Indiana
Purpose and Scope:
Provide nutritional services to patients at 2 dialysis clinics located in Columbus and Greensburg, Indiana suffering from kidney failure and disease in order to maximize the patient's nutritional status and improve clinical outcomes. Will operate as a member of an interdisciplinary healthcare team, supporting the FMCNA commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities.
Principal Duties and Responsibilities:
- Assessing patient's knowledge of diet and kidney disease and providing education appropriate to patient's learning needs regarding the relationship of diet and kidney disease and the impact on the treatment process.
- Providing patient-specific detailed education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status, and all nutrition related lab parameters pertaining to their disease process.
- Completing nutritional assessments and participating in interdisciplinary comprehensive assessments.
- Participating with the interdisciplinary team in the development of a written, individualized comprehensive plan of care that specifies the services necessary to address the patient's needs, in accordance with established guidelines.
- Calculating diet prescription according to standards of practice.
- Completing progress notes on all chronic dialysis patients according to established FMCNA guidelines and maintaining complete and accurate documentation of all appropriate information in the medical record according to established FMCNA policies and federal and state regulations.
- Providing patients with ongoing nutrition assessments and outcome-oriented nutrition counseling necessary to assist patients in achieving and sustaining an effective nutritional status.
- Identifying malnourished patients as well as patients at risk for malnutrition and working collaboratively with interdisciplinary team to identify appropriate interventions, resources or solutions.
- Monitoring adherence and response to nutrition therapy, adjusting therapy as needed to achieve and sustain an effective nutritional status, documenting why the patient was unable to achieve the goals and implementing the plan of care to address issues impacting the patient's ability to achieve the goals.
- Collaborating with the interdisciplinary team on management and recommendations for mineral metabolism and renal bone disease, anemia and iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation status and sodium/volume control.
Education Experience and Required Skills:
This position requires a Bachelor's degree in Nutrition or Dietetics and 1 year of experience as a hospital or medical clinic-based Dietitian. Must also be a Registered Dietitian as per the Commission on Dietetic Registration and hold a current state Dietitian licensure.
Position also requires 1 year of experience (which can have been gained concurrently with the primary experience requirement above) with: (i) renal population, dialysis/CKD 5; (ii) dietetic responses to mineral metabolism and renal bone disease management; (iii) identifying and addressing malnutrition; (iv) collaborating with an interdisciplinary team in regard to bone mineral disease, anemia, malnutrition, psychosocial status, modality, and sodium/volume control; and (v) completing comprehensive nutrition assessments and plans of care.
Must have excellent communication skills with physicians and inter-disciplinary medical teams.
Note that this position works at 2 dialysis clinics located approximately 25 miles apart in Columbus, Indiana and Greensburg, Indiana over the course of each week on a schedule determined by the manager. Candidate will be responsible for traveling between these 2 locations.
