About the job:
Clinical Case Manager utilizes various case management modalities in providing case management services to victims of crime and trauma, their families and significant others, in an atmosphere sensitive to each person’s physical, emotional, social and spiritual needs. The Clinical Case Manager uses professional and clinical judgment for initial and ongoing case management assessment, treatment planning, specialized trauma care and discharge planning; carries a select client caseload; supports the program as otherwise assigned by the program coordinator; practices within the scope of MCHS and department policies and procedures; practices within the scope of applicable registration or licensure under the Counselor, Social Worker & Marriage and Family Therapist Board of Ohio.
What you'll do:
Clinical Practice
- Completes case management assessments and determines appropriate level of care for new and returning clients.
- Demonstrates knowledge of psychological disorders and treatment modalities.
- Uses clinical judgment in establishing individual treatment plans and interventions, as indicated.
- Facilitates group therapy treatment as assigned.
- Orients clients to case management and effectively engages them in the process.
- May provides supervision for graduate students, interns, LSWs, PCs, MFTs with clinical assignments in the program.
Education, Outreach and Advocacy
- Provides consultation services to schools, courts, local businesses, and community providers.
- Initiates outreach opportunities, provides educational presentations and attends task force meetings as assigned.
- Serves as interdepartmental and interagency liaison.
- Performs personal advocacy, as requested by client, with other professionals and the criminal justice system.
Documentation/Reporting
- Documents client data accurately and completely, in accordance with documentation guidelines.
- Completes documentation/reporting on all activities as required.
Computer Skills
- Utilizes hospital computer systems effectively.
- Uses intranet and internet appropriately.
Initiative
- Participates in performance improvement
- Manages own productivity according to department productivity requirements.
- Participates in committee/project work, and unit activities.
- Orients new personnel.
- Takes responsibility for continuing education
What we're looking for:
- Minimum Education Requirement: Completion of Masters program in Social Work or similar degree
- Licensure Requirement: Independent licensure required by the State of Ohio Counselor and Social Work Board (LISW, LPCC, IMFT)
- At least two years of experience with case management, preferably working with trauma victims.
- Awareness of specific issues related to trauma and victimization
- Bi-Lingual preferred
- AOD experience preferred
