Your Role
The Blue Shield Promise Health Clinics team is responsible for providing comprehensive, communicative, and collaborative healthcare to our members that is of the quality that we would want for our own family members . The Licensed Clinical Social Worker- Behavioral Health will report to the Clinic Medical Director. In this role you will work with primary care providers to ensure that the medical and psychosocial needs of each patient are addressed. Our goal is to improve patient health outcomes through quality care and integrated caseload-focused consultation. The Licensed Clinical Social Worker – Behavior has the primary responsibility for responding to the clinical and psychosocial needs of patients referred by Primary Care Physicians and/or Social Services Supervisor. Blue Shield Promise Health Plan Antelope Valley Clinics serves individuals, couples, families, and groups from infants to adults. The Licensed Clinical Social Worker uses strengths-based wellness, resilience, and recovery models in conceptualizing the health and healthcare of members. Promise Health Plan clinicians are expected to be flexible in function and role; able to effectively adapt in a fast-paced, non-traditional clinical setting using developmentally and culturally sensitive best practices.
Your Work
In this role, you will:
- Collaborate with Primary Care Providers, case managers and other team members to complete member assessments related to social and resource needs to support member access to services.
- Collaborate with service coordination staff and act as liaison to ensure timely and accurate response to customer need.
- Collaborate with providers to support members treatment plan and care needs.
- Recognize the clients right to self-determination as it relates to the ethical principle of autonomy, including the client/family's right to make informed choices that may not promote the best outcomes, as determined by the healthcare team.
- Assess members health behaviors, cultural influences and clients belief/value system and evaluates all information related to current/proposed treatment plan and in accordance with clinical practice guidelines to identify potential barriers.
- Addresses referrals from providers including referrals via warm hand-offs.
- Assesses patients using standardized assessment/screening tools, measures, etc.
- Participates in care coordination with internal and external resources.
- Provides crisis assessments/triage with appropriate interventions.
- Participates in daily huddles to assess needs and coordinate services.
- Maintains daily scheduled appointments in our electronic records systems.
- Provide therapeutic services to patients and their families.
- Work in collaboration with multidisciplinary team members including external partners to perform other related activities designed to provide integrated health care services.
- Completes required documentation and paperwork including biopsychological assessments and treatment planning and implementation in a timely manner.
- Participate in Provider’s Meetings
- Demonstrates cultural competency and commitment to patients.
- Demonstrates commitment to the philosophy of the organization.
- Maintain case load of 25 to 30 patients weekly, and a minimum of 5 patients a day.
- Effectively communicates with all staff, providers, and community partners.
- Provide patient advocacy as needed.
Your Knowledge and Experience
- Requires a master's degree in Social Work or equivalent field
- Requires an LCSW
- Requires at least 5 years of prior relevant experience including 3 years of experience working with the Medi-Cal population, medical social work, community clinics, or other home and community based organizations
- CPR and first aid certification required
- Bilingual English/Spanish is desired but not required.
- Knowledge of eClinical Works (EMR) is desired but not required.
Pay Range:
The pay range for this role is: $ 77990.00 to $ 116930.00 for California.
