Job Summary and Qualifications
Weekend /Holiday Rotation and Minimum required for PRN
The Clinical Liaison contributes to high-quality, patient-centered care by identifying candidates for inpatient rehabilitation and initializing timely, appropriate patient transfers. In collaboration with the Rehab Medical Director, the Liaison ensures appropriate patient placement through clinical assessments and screening. The Liaison serves as an inpatient rehabilitation expert between internal and external referral sources and inpatient rehabilitation operations. The Clinical Liaison is responsible for establishing relationships with case managers, physicians, payors, and other referral sources.
What you will do in this role:
- In this PRN role, you will be floating to different facilities in the TriStar division as needed for onsite coverage.
- You will participate in Weekend/Holiday rotations and be responsible for meeting rotatation/minimum requirements.
- You will assist and coordinate intake and pre-admission screening processes. You will document evidence of rehab admission criteria within established timeframe.
- You will perform on-site and off-site clinical evaluations to assist Rehab Medical Director in establishing patient eligibility for admission.
- You will provide information to patient/family/caregiver related to inpatient rehab and program features. You will communicate expectations and requirements for participation in the rehab program and conduct or coordinate tours of the rehab unit.
- You will obtain information relating to patient resources and healthcare insurance benefits.
- You will demonstrate clinical knowledge of rehab admission criteria to appropriately identify and qualify rehab patients.
What qualifications you will need:
- Bachelors Degree required
- Speech Therapist, or Licensed Physical Therapy, or National Certified Occupational Therapist (NBCOT), or Registered Nurse (TN or Compact Lic)
