ResponsibilitiesAdult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, and in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky’s talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to the individuals served.

The Substance Use Counselor will enroll, screen, assess, and reassess people served for substance use, addiction risk, and addiction treatment needs. They also identify and evaluate risk issues, develop appropriate critical need interventions, and transition plans. The Substance Abuse counselor also works collaboratively with the team to provide clinical consultation and assist the team in understanding how the use of substances and/or addictive behavior can impact a person’s recovery
Other Key Responsibilities:
- Develop and facilitate clinical and support groups based on the needs and preferences of Persons served.
- Cultivate mutually respectful partnerships, set appropriate limits, and maintain a helping role with people served and family members.
- Coordinate with Care Coordination Entities and other providers to ensure that Persons have access to services that address addiction treatment and related issues. Provide direct interventions and supports to people served based on their stage of treatment or stage of change using evidence based and promising treatment practices.
- Develop productive affiliations and partnerships with other community-based providers to enhance the agency’s service delivery network, ensuring people have appropriate access to addiction services as needed.
QualificationsLADC I, LADC II, or CADC Required. Experience working with adults with mental health challenges, emotional disturbance, and co-occurring substance abuse disorders. Valid driver’s license and acceptable driving record.
