The Field Based Community Health Worker will act as a liaison role with Medicaid members and will be responsible for assessments, planning and implementing care strategies that are individualized by patient and/or Community Organizations and directed toward the most appropriate, least restrictive level of care. They also Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services, and manage the care plan throughout the continuum of care as a single point of contact for the member.
As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. The coordinator also addresses social determinant of health such as transportation, housing, and food access. Interesting in learning to work with medically complex patients who may be experiencing significant addiction and/or behavioral health conditions is important for this role.

Working Schedule: Schedule is Monday through Friday between the hours of 8am to 5pm. This position is a field-based position with a home-based office. You will work from home when not in the field.
Location: Suffolk County. Local travel up to 50% and mileage is reimbursed at current government rate.
Primary Responsibilities:
- Create a positive experience and relationship with the member
- Help member set goals and develop a care plan to achieve those goals
- Proactively engage the member to manage their own health and healthcare
- Support the member to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physician
- As needed, help the member engage with mental health and substance use treatment
- Support the member to ensure pick-up of their prescriptions
- Provide member education on community resources and benefits
- Conduct post discharge activities from hospital facility and support connection to social services
- Support member to engage in work or volunteer activities, if desired, and develop stronger social supports through deeper connections with friends, family, and their community
- Partner with care team (community, providers, internal staff)
- Knowledge and continued learning of community cultures and values
- May conduct Health Risk Assessments (HRA) if needed
- Performs other duties as assigned

Required Qualifications:
- High School Diploma / GED (or higher) OR 4+ years of community outreach experience
- 2+ years of experience working within the Community Health setting in a field-based healthcare role
- 2+ years of experience working within the local community, with demonstrated knowledge of culture and values within the community and familiarity with the resources available within the community
- Intermediate level of computer proficiency including MS Word, Excel, Outlook, and web applications
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
- Must reside in Suffolk County, New York
- Access to reliable transportation and the ability to travel locally approximately 50% of the time and up to 60 miles round trip to client and/or patient sites within a designated area
- Current and non-restricted state of New York driver’s license and state-required insurance
- Live in a location that can receive an approved high-speed internet connection or leverage an existing high-speed internet
Preferred Qualifications:
- Bachelor's Degree in a health-related field or social work or Health Care Administration
- Community Health Worker (CHW) State course completion
- Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant
- Background in managing populations with complex medical, behavioral needs and substance use disorders
- Experience with arranging community resources
- Experience with electronic charting
- Bilingual – Spanish
- Knowledge of Medicaid population
- Experience working in team-based / managed care
Physical Requirements:
- Ability to transition from office to field locations multiple times per day
- Ability to navigate multiple locations/terrains to visit employees, members and/or providers
- Ability to transport equipment to and from field locations needed for visits (ex. Laptop)
- Ability to remain stationary for long periods of time to complete computer or tablet work duties
Soft Skills:
- Ability to work independently and maintain good judgment and accountability
- Demonstrated ability to work well with health care providers
- Strong organizational and time management skills
- Ability to multi-task and prioritize tasks to meet all deadlines
- Ability to work well under pressure in a fast-paced environment
- Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others
