The social worker will be responsible for identifying and resolving psycho-social and financial needs of ill patients and their families and for providing resources to help with identified emotional, mental, and financial support when needed.
The hospice social worker is a key member of our interdisciplinary team that provides care to our patients and their families in their homes. Our organization is committed to your development and will train you in the tools needed to be successful in our organization The social worker will maintain required documentation in each patient's medical record in a timely manner. The social worker will represent the agency in a positive manner to the community, physician, and hospital staff and will act as an informant and referral source to the patient, physician, and community. The candidates should hold a master's degree in social work, or a bachelor's degree with experience, and current valid social work certification and having at least one year's experience in the home health or hospice setting is preferred.
The candidate must possess a high degree of initiative, enjoy working with elderly and ill people, and have excellent communication skills.
