Summary: Case Managers engage patients and their families in developing a safe discharge plan. Case Managers link the progression toward discharge with the clinical improvement of the patient.
Essential Functions
1. Coordinates clinical care to include discharge planning, medical necessity, length of stay management
2. Collaborates with providers, nursing, ancillary staff, patients and families to achieve optimal clinical outcomes
3. Executes clear, complete and concise documentation of all interventions and assessments
4. Engages in practice that comply with regulatory standards, patient safety and performance improvement initiatives

Required Education:
Bachelor’s degree is preferred
Master's degree is preferred
Required License:
Registered Nurse or Licensed Practice Nurse in North Carolina is required (or compact state)
