Job Summary
Help make an impact on the health of Minnesotans by providing leadership and supervision as a State Program Administrator Director in the Health Promotion and Chronic Disease (HPCD) Division as the Community Health Worker (CHW) initiatives and Engagement Director. This position will lead a new Division-wide focus on CHW initiatives and collaboration, partnership, and engagement with internal and external stakeholders to advance community engagement, clinical-community linkages, and quality improvement.
Responsibilities for the position include, but are not limited to, the following:
- Engage and strengthen partnerships with community, health system, local public health, and other stakeholders.
- Supervise the work of CHW Program staff.
- Design, Implement and evaluate initiatives.
- Provide technical assistance, consultation, and communication to outside agencies and organizations across the State and nationally.
- Grant management.
Work may be conducted in-person in St. Paul, or as telework for applicants who reside in Minnesota, Iowa, Wisconsin, North Dakota, or South Dakota, based on the business needs of the unit and supervisor discretion. Position location will be determined by the top candidate and will be based out of the closest Minnesota Department of Health office location. Occasional travel to the St. Paul office location will be required in addition to travel for purposes such as site visits and meetings with partners and community engagement activities.
Qualifications
Minimum Qualifications

Four (4) years professional experience in public health and/or chronic disease. Experience to include all of the following:
- Strategic planning, project management, community engagement, and meeting facilitation.
- Establishing working relationships with community members and planning groups.
- Oversight of grants and contracts.
A degree in public health, epidemiology, community health, nursing, medicine or related field may count towards professional experience as follows:
- Bachelor's = 12 months
- Master's = 18 months
- Doctoral = 24 months
Abilities and Skill to be assessed at interview:
- Ability to synthesize new information and make decisions on program and policy actions.
- Ability to work independently and as part of a team.
- Ability to lead, motivate, and guide staff to work effectively with stakeholders to achieve goals.
- Ability to explain the role of community health workers to stakeholders.
- Communication skills sufficient to effectively present, written and spoken ideas and reports, to stakeholders.
Preferred Qualifications
- Advanced degree in public health, community health, environmental health, nursing, epidemiology, social work or related field.
- Experience in the community health worker profession, systems, stakeholders, and models.
- Experience in managing a statewide program.
- Experience managing multiple overlapping projects that engage a variety of stakeholders.
- Experience evaluating program performance.
- Experience with policy and systems-level change strategies.
Additional Requirements
- This position requires successful completion of a background check.
- This employment does not require a valid driver's license, but it does demand occasional travel, which the candidate will be responsible for meeting.
- The successful candidate under U.S. Citizenship and Immigration Services regulations must be able to accept work in the U.S. by the day employment begins. The agency is unable to offer H-1B sponsorship.
