Job Summary
Are you a lifelong learner looking for an opportunity to analyze and assess, problem solve, and lead? Health Care Homes (HCH) is looking for a Practice Improvement Specialist to join their collaborative team! This position provides practice transformation technical expertise and leadership for HCH certification teams and primary care clinicians and clinics.
Primary responsibilities include:
- Leads the HCH certification teams in planning and conducting certification site visits and subsequent follow-up, ensuring compliance with HCH standards and criteria in the health care homes rule.
- Provides coaching, facilitation, and technical consultation to primary care clinicians, clinics, and community partners, helping them achieve transformation and quality improvement goals through HCH certification.
- Advocates for the healthcare homes program among uncertified primary care clinics and other stakeholders, while contributing to various activities involved in program implementation.
This position is eligible for hybrid telework to applicants who reside in Minnesota, Wisconsin, Iowa, North Dakota or South Dakota. Limited travel within the metro region to assigned primary care clinic sites at less than 10-15% is required for the position.
This position works 32 hours/week. There is flexibility with the schedule that is at supervisory discretion.
Qualifications
Minimum Qualifications
- Possession of a current Registered Nurse's license in Minnesota, without restriction.
- Two (2) years of experience working as a Registered Nurse in a primary care or community health setting.
- A degree in Nursing, Public Health, Health Care Administration, or a closely related field may count towards professional experience as follows:
- Associates = 6 months.
- Bachelor's = 12 months.
- Master's = 18 months.
- PhD = 24 months.
Minimum qualifications that will be assessed during the interview process:
- The ability to establish and maintain credibility with primary care practices, community partners, and the public by using interpersonal, communication, and facilitation skills.
- Knowledge of clinic performance management and quality improvement, including the ability to analyze data.
Preferred Qualifications
- Knowledge or experience of patient centered medical homes.
- Knowledge of health disparities and social determinants of health, and experience applying principles that advance health equity.
- Experience leading or managing projects or teams.
- Experience working collaboratively across community partners to advance population health.
Additional Requirements
This position requires successful completion of a background check and employment reference check.
This employment does not require a valid driver's license, but it does demand travel, which the candidate will be responsible for meeting.
