Westminster Canterbury of Lynchburg, a 5-Star rated Continuing Care Retirement Community in Central VA is seeking a Hospice RN Case Manager to join our team! Full-time position available with complete benefits package including Medical, Dental, Vision, Paid Time Off, 403B with match, and more. Hours are typically Monday through Friday but will require some on-call and weekends on a rotating basis.
The RN Case Manager is responsible for the planning, coordination and delivery of quality health care services, assuring compliance with corporate standards and federal, state and local regulations. Demonstrates knowledge and support of the philosophy, purpose, goals and policies of Senior Independence and consistently complies with the Client Bill of Rights.
- Responsible for the implementation of the plan of treatment, by executing, organizing, overseeing, integrating, and modifying the resources necessary to reach desired outcomes. Evaluates the effectiveness of the plan on an ongoing basis. Participation in the IDG.
- Provides ongoing assessment, nursing care and teaching to clients in the home, assisted living facilities and long-term care facilities. Assures an accurate evaluation of client needs and develops and implements a comprehensive care plan that addresses those needs. Follows current home health and/or hospice nursing professional standards of practice and agency approved protocols, including disease management programs for diabetes, heart failure, chronic obstructive pulmonary disease, and other commonly treated diagnoses.
- Maintains current and accurate client records through use of laptop computers and paper/chart documentation in accordance with corporate policies, federal, state, and local guidelines.
- Collaborates with other disciplines, suggests and/or processes internal or external referrals in order to meet the comprehensive needs of the clients.
- Supervises and monitors the performance and skills of Home Health/Hospice Aides, and/or Licensed Practical Nurses, assuring the delivery of quality health care services.
- Maintains knowledge of requirements of regulatory agencies, accrediting bodies, and third party payers.
Requirements:
- A Valid unencumbered VA RN license
- Prior Hospice or Home Health Experience is preferred.
- Current CPR certification (Basic Life Support)
- Have a valid Virginia Driver's License with a good driving record
