POSITION TITLE: Case Manager-Warren City Schools
DEPARTMENT/PROGRAM: Valley Counseling Services
REPORTS TO: Clinical Director or Supervisor
SCHEDULE: The standard work week is 40 hours with the regular school hours of 7:30 am - 4:00 pm, however, exact working hours are dictated by client and agency need. Evening hours may be required. Summer hours are Monday through Friday 8:00 – 5:00pm. Summer hours will be scheduled with supervisors.
GENERAL STATEMENT OF DUTIES: Provides behavioral health interventions, behavioral modification strategies, skill building, and resources to child and/or adolescents within the school setting. This position is funded in part by the Warren City School District.
ESSENTIAL RESPONSIBILITIES:
1. Provide behavioral interventions and support to school aged students in their respective educational setting to assist them in developing and maintaining effective and productive behaviors.
2. Assist administrative and educational staff by providing consultation services related to the client in the areas of behavioral modification, crisis resolution, and healthy social/emotional development.
3. Monitor and coordinate the delivery of necessary evaluations (psychiatric, medical, vocational, health, etc.) and assessments in a timely manner to ensure that client's needs are adequately identified.
4. Utilize Trauma Informed care when appropriate with client/students.
5. Follow program curriculum, training and guidelines at all times.
6. Make appropriate referrals to services needed in conjunction with client/student, parent/guardian and school personal.
7. Develop Individual Service Plans that assist in achieving success in the educational setting.
8. Assist the individual in achieving ISP objectives.
9. Maintain productive and professional relationships with school personnel and other agency staff.
10. Work effectively within a school setting following their rules and regulations and following the lead of the school administrator when necessary.
11. Document all encounters in Electronic Health Record. Utilize appropriate documentation for billing and encounters. Maintain 50% productivity monitored through appropriate documentation.
12. Attend all trainings required by school, and AxcessPointe and/or any other required training.
13. Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.
14. Job assignment is contingent upon partnering with a school and can be severed by the school or AxcessPointe at any time. Continued employment is at the sole discretion of the employer.
15. Perform other assigned and related duties specific and customary to the department in which they are assigned.
QUALIFICATIONS:
1. The position requires the skills and knowledge in the areas of behavioral modification, behavioral health issues, de-escalation techniques, community support and the educational process.
2. The position also requires the ability to respond appropriately to crisis or emergency situations.
3. The position requires the ability to communicate effectively verbally and in writing; the ability to establish and maintain effective relationships with clients, their families, educational personnel, and agency staff.
4. Ability to work independently as well as cooperatively, reliably and dependably, and the ability to manage available resources to carry out assigned tasks.
5. Must be able to meet agency requirements for criminal back ground check (Ohio Bureau of Criminal Identification & Investigation).
MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS:
Bachelor's Degree in a mental health or education related field preferred.
MINIMUM EXPERIENCE REQUIREMENTS: Prior behavioral health experience preferred. Prior educational experience preferred.
Time off Scheduling:
School-based employees will follow the school schedule as assigned.
Eligible school-based employees will also accrue Vacation time, which can only be used during the Summer while school is out of session.
