Responsibilities of the MSW/Social Worker:
The MSW supports and maximizes the psychosocial functioning and adjustment of patients who are experiencing life changes and/or end-stage disease. These services are provided to ameliorate social and emotional stresses resulting from the interacting physical, social, and psychological concomitants of end-stage disease, including shortened life expectancy, altered lifestyle, and demands of a rigorous, time-consuming, and complex treatment regimen. MSW integrates care management, social services, discharge planning, and pre- and post-hospital services to ensure clinical efficacy and best outcomes for our patients. This position ensures the provision of quality health care along the continuum of care, decrease fragmentation, enhance the patient's quality of life, efficiently use patient care resources, maximize cost containment opportunities, and improve successful post-hospitalization transition care. This position reports to the department head. MSW guides the integrated team in the functions of care coordination, facilitation of referrals, education, discharge planning, and advocacy.
Requirements of the Counselor III
