The RN Case Manager performs the primary function of a Registered Nurse in assessing, planning, implementing, and evaluating the care of patients in the Wound Healing Center.
- Assess, stage, and measure wounds accurately and in accordance with policy. Assists physician with wound therapy. Performs assessments and documents ongoing care plans.
- Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs. Provides patient education.
- Participates in quality improvement opportunities.
