ESSENTIAL JOB RESPONSIBILITIES:
- Complete home visits as required by the funding source and as assigned by the supervisor.
- Generate required units, complete thorough and detailed documentation of services provided, and complete reports as required by funding source, policy, and as assigned by supervisor.
- Conduct necessary assessments, complete legal paperwork and complete Individualized Service
- Recovery Plans (ISRP) according to funding source timelines, policy timelines and as assigned by supervisor.
- Provide services as identified on ISRP.
- Plan Schedule, coordinate meetings with participants and other providers.
- Research, access and link to appropriate medical, psychiatric, education, community resources and other social services as indicated by assessments and individual participant preferences.
- Communicate and work collaboratively with other providers to ensure appropriate service provision.
- Meet with the supervisor for regularly scheduled staff meetings and as assigned by the supervisor.
- Attend and actively participate in regularly scheduled staff meetings and as assigned by the supervisor.
- Attend and actively participate in required training and training as assigned by the supervisor.

REQUIRED SKILLS:
I. PROVISION FOR CASE MANAGER SERVICES:
A. Work with the participant, his/her identified support systems to set long-range and short-range
goals based on the ISRP, ensuring professional recommendations including but not limited to
physician, psychiatrist, counselor/therapist(s) and other service provider(s) are incorporated into
goals and outcomes.
B. Ensure delivery of services meets the established intensity and frequency as indicated by ISRP,
funding source or supervisor assignment.
C. Ensure performance benchmarks are met or exceeded for units generated.
D. Ensure documentation and required reports are completed within timelines as required by funding
source, policy and supervisor.
E. Makes referrals to appropriate medical, psychiatric, education, community resources and other
social services agencies and/or providers in a timely manner to address identified needs.
F. Document and follow-up in a timely manner to ensure services are in place and appropriate.
G. Establish and maintain ongoing rapport with participants and his/her identified support systems to
assist participants in achieving his/her identified goals.
H. Establish and maintain ongoing rapport with referral sources, other community agencies and
providers to coordinate services for participants.
I. Respond to referrals and requests for services within timelines as required by funding source,
policy and supervisor.
J. Empower and encourage participants and his/her identified support systems to be active partners
in services.
K. Ensure all participants, and their significant others are treated with respect and dignity; and to
ensure their rights are preserved.
L. Maintain a minimum 40-hour work week.
M. Actively participate in case reviews, team meetings, staff meetings and other trainings as required
by funding source, policy and supervisor.
N. Communicate and update your Supervisor or PM regarding any training or consultation needs
related to service delivery, performance expectations or documentation requirements.
II. CLIENT FILE MAINTENANCE
A. Records, plans, legal forms and other documentation are kept accurate and up to date.
B. All services provided are documented and appropriately signed and dated and in the appropriate
location in the chart based on funding source, policy and supervisor.
Documentation reflects current services, interventions, participant participation, progress towards goals
and/or any challenges or barriers to achieving goals.
Requirements
- Bachelor's Degree in Social Service or Human Services related field.
- Must have their own reliable transportation.
- Willing to travel during the day to outlying parishes as required.
- Able to multitask and meet deadlines.
- Excellent written and verbal communication skills.
- Proficiency in the use of Microsoft Office Software (i.e. Word, Excel) and computer literate.
- Must be a team-player.
- PHYSICAL REQUIREMENTS with or without reasonable accommodations:
- The employee is regularly required to operate a computer, file and retrieve written documents,
- communicate with others on the phone and in-person.
- The employee is frequently required to walk, sit, use hands, and lift and/or move lightweight items.
- Ability to move independently within the facility and community.
