JOB OVERVIEW
Odom Rehab, a division of Odom Health and Wellness ("OHW"), is an emerging leader in innovative senior rehabilitation, providing top quality rehab to our clients. The Physical Therapist will be responsible for providing outpatient physical therapy services for Odom Rehab in an ethical and professional manner. Physical Therapy services for Odom Rehab will be provided in accordance with each specific state guidelines.
The employee shall also perform (i) such other duties as are customarily performed by an employee in a similar position, and (ii) such other and unrelated services and duties as may be assigned to the employee from time to time by Odom Rehab.
ESSENTIAL FUNCTIONS

Provide Physical Therapy Services
Evaluate and treat patients in accordance with state guidelines, identifying root causes of dysfunction that directly translates to impairments in daily function.
Establish appropriate POC (Plans of Care) for the patients outlining the path to achieve the patient's goals; reassess periodically to keep goals relevant.
Oversee the treatment provided by the PTA based on established POC.
Deliver treatment to patients including manual therapy, educating and performing therapeutic exercises, training of proper techniques for movements and body mechanics, developing balance systems, and addressing pain to reach goals.
Educate patients on best ways to self-manage conditions; and educate family on best ways to help the patient.
Update POC as necessary for the patient's care.
Assess appropriate adaptive equipment, including canes, walkers, orthotics, etc.
Develop a discharge plan for patients once completing their physical therapy.
Document patients' treatment daily based on established and accepted practice guidelines.
Submit appropriate charges daily for Physical Therapy services provided that date.
Communicate on a regular basis with the physician and medical staff regarding patients' status or change in status.
Assist in developing and updating ODOM REHAB treatment protocols on an as needed basis.
Actively participate in ODOM REHAB staff meetings.
Track and complete Continuing Education Credits and renewal steps necessary to keep current on Physical Therapy licensure.
Maintain an active Physical Therapy license.

CERTIFICATIONS:
Graduation from an accredited Physical Therapy program with a degree in Physical Therapy and MN State Physical Therapy Licensure
Recent / previous work experience in primary health care setting (preferred)

6. For Mandatory Administrative Drivers: Valid Minnesota driver's license, acceptable driving record, and access to a reliable vehicle required.
7. Must maintain personal vehicle auto insurance coverage.
WORKING ENVIRONMENT AND PHYSICAL REQUIREMENTS
Work Rating: Medium Work
_Medium work. _Medium work involves lifting no more than 50 pounds at a time with frequent lifting or carrying of objects weighing up to 25 pounds. If someone can do medium work, we determine that he or she can also do sedentary and light work.
DRIVING REQUIREMENTS:
This position has been designated as Mandatory Driving: Administrative.
As part of the new hire onboarding process OHW conducts background checks including a review of driving records for all employees required to drive in their position. In order to be in good standing, you will need to maintain a satisfactory driving record. OHW will assess individual driving records to determine if a person meets OHW's driver safety requirements. If an individual's driving record indicates a violation or combination of violations that are deemed excessive, OHW may discipline the employee up to and including termination of employment.
