Location: Queens, NY
Compensation: $95,000 - $110,000/year
Schedule: 9:00 AM to 5:00 PM, Monday to FridayResponsibilities as the Nurse Care Manager (MLTC): - Care Coordination: Provide care coordination by conducting in-home assessments, planning, and facilitation to meet members health needs.
- Member Collaboration: Work closely with members, families, caregivers, primary care practitioners, and the interdisciplinary team to maintain independent living situations.
- Service Authorization: Authorize covered services and coordinate care across all payers.
- Health Monitoring: Assess and monitor members' needs and status to ensure consistent care across the healthcare continuum.
- Communication: Collaborate and communicate effectively with all stakeholders involved in the member s care.
Qualifications for the Nurse Care Manager (MLTC): - Licensure: Must possess a current NYS RN License and registration.
- Education: Bachelor of Science degree in Nursing required.
- Experience: Minimum three (3) years of RN experience; clinical expertise in geriatrics and managed long-term care experience preferred.
- Language Skills: Bilingual Spanish required!
- Additional Skills: Strong communication, collaboration, and advocacy skills.
