Oneida Health's Extended Care Facility (ECF) is a 160-bed skilled nursing and rehabilitation center that strives to provide the highest level of quality care for residents. Physical Therapist, PT is responsible for evaluating, planning, & providing physical therapy treatment to patients in accordance with written prescription from a qualified provider
Assure that the highest degree of quality resident care is always maintained
In the case of Direct Access, the physical therapist will evaluate, plan, & provide treatment within the guidelines set forth regarding Direct Access.
Physical Therapist, PT will participate in discharge planning, development & implementation of resident care plans, resident assessments (MDS), etc. as necessary
Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act as well as Physical Therapy Aides & clerical employees assigned to the service
Maintain treatment records, resident files, & progress notes as required; adequately liaise with families & residents
Requirements
Current license & registration to practice as a Physical Therapist, PT in the State of NY
Master's Degree in Physical Therapy (Preferred)
Bachelor's Degree & relevant experience may be considered
Physical Therapy Experience: 1+ years (Required)
Energetic, enthusiastic, committed, & passionate about quality care
Interested in helping variety of patients of all ages who are ill, injured, &/or disabled
Excellent communication, customer service, & organization skills
Able to demonstrate patience & empathy
Trusted to work independently & in a team environment
Work well under pressure & will work harmoniously with all staff
Good knowledge of anatomy and physiology, neurology & socioeconomic factors affecting a patient's disabilities
Computer skills relative to electronic records, charting, & auditing
BLS/CPR Certification
