Optum Care Network Washington is seeking a full-time Nurse Practitioner - Population Health Education & Clinical Performance works collaboratively to educate on and improve accurate coding and documentation skills, leading to a more complete patient record, as well as other value-based care initiatives.

Primary Responsibilities:
- Provides education to providers on coding and documentation for Medicare risk adjustment, quality, patient experience and affordability topic
- Review charts and performance data to aid in the education process and discover opportunities to improve accurate coding and documentation
- Develops and updates training materials, presentations, and job aids
- Meets with Care Delivery and Network Medical Directors to assist in identification of operational and clinical best practices in maximizing patient visits, re-evaluation rates and accurate and proper coding; assists in the dissemination of best practices to sites, clinicians and IPAs providers / support staff
- Floating to various clinics to see patients. Key elements of the visits include:
- Address quality care gaps
- Identify and address factors related to chronic conditions that may not have been recognized
- Educate patients about resources available to them within OptumCare Network
- When necessary, link patients to care management or other services

Required Qualifications:
- Current Washington state license
- Nurse Practitioner Certification (NP-C)
- Primary care experience, preferably managing a panel of patients
- Proven excellent communication skills, high patient satisfaction and demonstrated experience promoting teamwork
- Proven ability to build positive working relationships
Preferred Qualifications:
- Experience in value-based care contracts
- Experience in HCC coding
Washington Residents Only: The salary range for this role is $110,040 to $139,000 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
