JOB SUMMARY (Overview of job): Certified Home Health Aide's (CHHA) main role is to provide direct patient care in the home or setting where the patient refers to as home. Certified home health aides perform basic duties such as; bathing, assistance with activities of daily living, safety and vital signs. The CHHA must demonstrate competency with all core skills as required by the Certified Nursing Assistant licensure. Demonstrates effective verbal and written communication skills. Understands basic knowledge of ethical issues and is able to provide care to chronically and acutely ill patients. Provides caring holistic person-centered care. Demonstrates knowledge of basic infection control practices; such as hand washing technique. The CHHA is under supervision of the Home Health clinical manager/supervisor and/or director. Ability to communicate with the RN or PT/OT/ST case manager for concerns that may arise in the home while performing patient care. On a daily basis communicate with the CHHA scheduler. The CHHA follows an individualized plan of care that is developed by the Home Health RN/PT/OT/ST case manager. The CHHA will demonstrate an understanding of the plan of care prior to the first visit with patient. Maintains HIPPA compliance. Has a general understanding of regulatory compliance to assure the provision of safe, quality care.
KNOWLEDGE/EXPERIENCE: Sufficient knowledge, effective communication skills, and experience to demonstrate ability to work autonomously. Must demonstrate professionalism while working with patients in their homes. Prefer at least one year of long-term care experience or prior home health or hospice experience.
EDUCATION: High school diploma or GED preferred.
LICENSE/CERTIFICATION/REGISTRY: Current Montana License as a Certified Nursing Assistant (CNA) required, BLS required.
