Evaluates and treats patients with different diagnosis in accordance with physician's orders.
Initiates skilled physical therapy treatments in accordance with physician's orders.
Supervises physical therapist assistants and rehab technicians.
Completes and maintains required documentation such as evaluations, progress notes, daily notes, recertifications, and discharge summaries for patients on caseload.
Identify and meet patients' goals and needs.
Offer cost-effective treatments that help improve clients' motion and mobility.
Reduce the need for medications and provide alternatives to surgery.
Develop care plans using a variety of treatment techniques.
Create fitness- and wellness-oriented programs tailored to patients' specific needs.
Provides quality, personalized and evidence-based care, and proven interventions.
Motivates patients during treatment to help them function optimally.
Promotes patients' healthy lifestyle by improving strength, flexibility, balance, and coordination.
Consults and practices with other health professionals regarding special needs of patients to enhance rehabilitation or maintenance of function.
Provides input for the Care Planning meetings and attends during absences of the DOR.
Maintains statistical records of residents' treatments.
Assists with in-service training programs upon request.
Submits requisition for supplies needed in the treatment of residents to DOR.
Assumes all duties of the Director of Rehabilitation, if applicable, during absences including supervision of rehab technicians and responsibility for all necessary documentation.
Arrives to work on time, regularly, and working as scheduled, responsible for own transportation.
Observes and practices confidentiality of resident information and privacy when performing job duties.
Recognizes and follows the dress code of the facility including wearing name tag at all times.
Performs assigned duties according to the service excellence standards and goals established for this position, and for all staff members of this facility.
Attends facility meetings, staff meetings, and required or preferred in-services.
Generates and develops ideas which improve the quality of care for patients in the facility.
Performs all position duties according to safety rules and practices required for this position, and as required to ensure the general safety of staff, residents and visitors of this facility.
Demonstrates knowledge and understanding of all policies and procedures and ability to reference them from appropriate manual.
Exhibits the highest level of customer service towards residents, staff and visitors.
Ability to work with the highest efficiency and time management.
Exhibits flexibility and consistently demonstrates the ability to respond to changing situations to meet facility and patient needs.
