Oregon Continental Home Health Care is a small home health company with a rapidly expanding patient base whose focus is on the underserved in the community. We are seeking a skilled and flexible PT to provide home care services to various adult populations in Multnomah and Washington county. The current need is Multnomah county, however flexibility is a must. Full-time employees are guaranteed 40 hours with drive-time included. Typical caseload is 20-25 visits a week (depending on the amount of visits/evals/OASIS) and PTs will schedule their own patients for increased flexibility within their work week. PTs will collaborate with other PT/PTAs, OT/COTAs, and RN/LPNs within the company to problem-solve and provide optimal care to patients.
Oregon Continental is offering a $1,500 sign on bonus for our full time employees, of which $500 paid after the first 90 days, $500.00 at 120 days and $500.00 at 180 days. Probationary evaluation at 90 days could earn you up to $2.00 extra on your hourly pay.
Oregon Continental offers Prorated PTO and sick time from your date of hire to December 31st and we front load 40 hours of PTO and 16 hours of sick time every January 1st. We also provide 1-week vacation on you hire anniversary date of employment, and another week at 2 years. $500 is rewarded at the end of the year to use as desired for CEUs, car maintenance, etc and a $50 gas bonus is given for full-time employees when a full schedule is seen.
Therapist are required to:
Devise detailed treatment and care plans for each of the patients
Communicate and coordinate with patients' physicians to ensure a consistent level of care
Visit patients in their homes
Exercise independent judgment based on experience to implement effective treatment combinations
Evaluate and report on patients' progress
Assist administrative staff with billing and reporting inquiries
Comply with federal and state physical therapy regulations
Complete at least 10 hours of continuing education every two years
Applicants must have reliable transportation, unencumbered Oregon License along with an active drivers license with car insurance.
