Summary:
To provide case management and education to populations of patients living with chronic disease. The RN Care Manager provides education and resources to patients across the continuum including the inpatient setting, outpatient areas and possibly in the home when needed. The RN Care Manager acts as an adjunct medical and allied health professionals in the community in caring for patients with chronic illness. Acts as a resource to primary and specialty physicians in identifying and managing high cost and high risk patients.
Minimum qualifications:
- A Bachelor's Degree in Nursing and licensed to practice nursing in the State of Connecticut
- 7 years of experience in acute care, home care or long term care
- Certification in a nursing specialty
- Advanced knowledge and understanding of the healthcare delivery system and chronic care populations
- Strong organizational, communication, problem solving and decision making skills.
- Specialty certification in COPD preferred and/or a minimum of 5 years experience in management of COPD.
