Avera is currently seeking Registered Nurses (RN) Pharmacy Case Manager

Job Summary
The specialty pharmacy case manager facilitates the pharmacy plan of care by working closely with the clinical pharmacist, physicians and payors to assure proper use of specialty medications in order to manage patient adherence, clinical outcomes and payor requirements. Performs scheduled assessments with patients according to applicable therapy management protocol and collaborates with the interdisciplinary care team to encourage the best possible patient outcome. Serves as a liaison with physicians and other referral sources to assure continuity of care.
Education and/or Experience
- Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.
- Bachelor's degree in nursing preferred.
Licensure/Certification/Registration
- Current and unrestricted licensed registered nurse in the State of South Dakota and/or meeting the requirements of multi-state unrestricted licensure required.
- Unrestricted registered nurse licensure in the State of Minnesota is required with-in 90 days.
- Certification in specialty preferred.
- Membership in a professional organization preferred.
Work Schedule
- Monday-Friday, 8:00am-5:00pm; 80 hours per 2 weeks

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
